CHANGING
MEDICAL AND DENTAL/VISION PLANS
In
most cases, you may change from one LACERA-administered health care plan to another
after you complete a six-month waiting period and have continuous coverage.
Want
to change plans?
1)
Contact LACERA and request a Medical Plan or Dental and Vision Plan Change
Form.
2)
Fill out the form, sign it, date it and mail it back to LACERA.
Coverage takes effect the first day of the month after completion of
a six-month waiting period. The waiting period begins when your
Change Form is received by LACERA. Your current insurance coverage
will continue until your new coverage becomes effective.
If you are covered by the Firefighters Local 1014 health plan and wish
to change to a LACERA-administered health care plan you may do so upon completion
of the six-month waiting period.

EXCEPTIONS TO THE SIX- MONTH WAIT
The general six-month waiting period is waived for certain specific situations.
- You move out of the designated service area of the Health Maintenance Organization (HMO) or Medicare HMO plan in which you are enrolled.
- You
are currently enrolled in the Anthem Blue Cross Prudent Buyer
Plan and move out of California.
- You
change your current non-Medicare health plan (Anthem Blue Cross
I, Anthem Blue Cross II, CIGNA Network Model, Anthem Blue Cross
Prudent Buyer Plan, Kaiser) into a LACERA-administered Medicare
HMO plan (Kaiser Senior Advantage, Secure Horizons, SCAN)
- You
change from Anthem Blue Cross I into Anthem Blue Cross II.
- You
change from Anthem Blue Cross I or Anthem Blue Cross II, into
Anthem Blue Cross III.
