DISEASE MANAGEMENT PROGRAMS
All LACERA-administered medical plans have Disease Management Programs as part of their benefits. Disease Management Programs offer support, tools and information to help LACERA members manage certain chronic conditions:

• Asthma
• Congestive heart failure
• Diabetes

Starting July 1, the Disease Management programs for all Anthem Blue Cross Plans included a new focus:

• Preventing and managing hypertension.

The Disease Management Programs offered by all LACERA-administered medical plans are voluntary and confidential – we encourage you to take advantage of them.
