CIGNA
NETWORK MODEL PLAN HMO
This
plan is a combined staff model and IPA HMO which is available to all eligible,
retired LACERA members and their dependents.
The CIGNA
Network Model Plan includes these features:
- Selection of a primary care physician from the CIGNA Medical Group
or a participating private care physician.
- All medical care must be received at a CIGNA Health Care Center if
you select a physician in the CIGNA Medical Group unless an emergency
occurs out of the service area.
- All your covered health care must be received from the same physician
if you select one that is in private practice, except in the event of
an emergency.
- You may change your primary care physician at any time during the
year.
- You pay a small copayment for most outpatient services.
- There are no deductibles, annual dollar maximums, and no claim forms.
- Health education classes are available.
