PLANS CHANGE WHEN YOU MOVE OUT OF STATE
Health care plans will probably change if you move out of state, or the country.  Contact the  insurance company for specific information about coverage differences.  Notify LACERA before you disenroll in your current health plan.

Anthem Blue Cross Plan I, II 
(Indemnity Plans)

Plan pays 80% of covered expenses after deductible has been met.  Deductibles and hospital benefits differ between Plans I & II.  USA coverage & many foreign countries.*    

Anthem Blue Cross Plan III 
(Indemnity Plan)

Plan pays 20% of Medicare approved charges (80% emergency).  USA coverage only.  Emergency coverage only in foreign countries.*

Anthem Blue Cross Prudent Buyer Plan

Plan pays 70% of covered expenses after deductible has been met (80% emergency).  USA coverage only.  Emergency coverage only in foreign countries.*

SCAN

Plan is available in the following counties in Southern California: Los Angeles County, Orange County, Kern, Riverside County, San Bernardino County, and Ventura County.

CIGNA HealthCare Network Model

Plan only covers California and Arizona.

CIGNA Medicare Select Plus Rx Available only in Phoenix, Arizona area.

Kaiser Permanente/
Kaiser Senior Advantage

Plan only covers some areas in California, Colorado, Georgia, Hawaii, and Oregon.

PacifiCare

Emergency coverage for 60 days outside of CA. 

Secure Horizons

Plan only covers California.

OUT OF THE COUNTRY MOVES
If you move permanently to a foreign country the only available plans to you are Anthem Blue Cross Plans I and II.  Anthem Blue Cross does not preauthorize any medical services provided in foreign countries.  You must pay first, then send in bills, proof of medical necessity, payment information, and an Anthem Blue Cross foreign Claim Form, for reimbursement.
