PacifiCare is
an IPA HMO available to retired, eligible LACERA members and their dependents
who are under
age 65
and not
eligible for Medicare.
An IPA model consists of physicians in private practice that contract
with an HMO to provide services to its members.

A
T T E N T I O N
PacifiCare
members must contact LACERA at least 3 months before their
65th birthday to enroll in Secure Horizons or another LACERA-administered
medical plan.
PacifiCare is only for those members
under age 65.

- Copayments are required for certain services
SECURE
HORIZONS
Secure
Horizons is an IPA HMO Medicare Advantage-Prescription Drug Plan (MA-PD) available to retired,
eligible LACERA members and their dependents who are age
65 or older
and enrolled
in Medicare Part A and B. If
you have Medicare coverage and enroll in Secure Horizons, all non-Medicare
eligible family members under age 65 must enroll in PacifiCare.
Secure Horizons is a subsidiary of PacifiCare. They contract with
physicians in private practice to provide health care services.

- Your premiums
for Medicare Part B are reimbursed by the County on a tax-free basis
- "Senior-Fit"
is a free health assessment and custom-designed fitness program

is a support program for enrollees, who are caregivers,
to use to find answers, services, and support to care for a relative,
