CARE ACTION NETWORK (SCAN) Medicare HMO Advantage-Prescription Drug Plan (MA-PD)
SCAN
is available to LACERA retirees
and their eligible dependents who are age 65 or older and under age 65
enrolled in Medicare Parts A and B.
SCAN Health Plan is not available to family members who are under age 65 unless
enrolled in Medicare Parts A and B.

SCAN offers
the "safety net" of Extended
Home Care Services
which include personal care services such as bathing and feeding, housekeeper
services, home delivered meals, emergency response, and adult day care.
SCAN also
offers qualified members Independent Living
Power™ and In-Home
Recovery Benefits, programs designed to help SCAN members remain in
their home as long as safely possible. The program is a collection
of personal care services ranging from light housekeeping to transportation
escorts.
SCAN Health
Plan now serves seniors with Medicare Parts A & B in Maricopa
County, Arizona. SCAN is also available in the following
counties in the state of California: Los
Angeles, Orange, Riverside, San Bernardino, San Diego, Kern,
and Ventura counties.
