DISEASE MANAGEMENT PROGRAMS
All LACERA-administered medical plans have Disease Management Programs as part of their benefits. Disease Management Programs are designed to help you make a positive difference in your life that offer support, tools and information to help you LACERA members manage certain chronic health conditions:
- Congestive heart failure
If you are enrolled in the LACERA-administered Cigna Medical HMO, Kaiser/Kaiser Senior Advantage, United Healthcare/United Healthcare Medicare Advantage, or SCAN Health Plan, call your carrier or visit their website listed under Contact Healthcare Insurance Companies.
Anthem Blue Cross Prudent Buyer Plan
If you are enrolled in the LACERA-administered Anthem Blue Cross Prudent Buyer Plan: Call Anthem Blue Cross Condition Care at (800) 522-5560.
Anthem Blue Cross Plans I, II, or III
If you are enrolled in the LACERA-administered Anthem Blue Cross Plans I, II, or III, beginning July 1, 2015, Accordant will replace Alere. The Alere Disease Management programs will no longer be offered.
Beginning September 1, 2015, CVS Caremark's Pharmacy Advisor will become available in addition to Accordant.
Beginning July 1, 2015, the Accordant program will provide no-cost services and support to manage the following health conditions:
- Amyotrophic Lateral
- Sclerosis (ALS)
- Chronic Inflammatory
- Crohn's Disease
- Ulcerative Colitis
- Cystic Fibrosis (CF)
- Gaucher Disease
- Multiple Sclerosis (MS)
- Myasthenia Gravis (MG)
- Parkinson's Disease (PD)
- Rheumatoid Arthritis (RA)
- Seizure Disorders (Epilepsy)
- Sickle Cell Disease (SCD)
- Systemic Lupus Erythematosus
- (SLE or Lupus)

Pharmacy Advisor Counseling
Beginning September 1, 2015, CVS Caremark's Pharmacy Advisor will become available in addition to Accordant. The Pharmacy Advisor is a program that provides personal pharmacy care for adults managing conditions like diabetes, high blood pressure, and heart disease
