DISEASE MANAGEMENT PROGRAMS
All LACERA-administered medical plans have Disease Management Programs as part of their benefits. Disease Management Programs offer support, tools and information to help LACERA members manage certain chronic conditions:
- Asthma
- Congestive heart failure
- Diabetes
Starting July 1, the Disease Management programs for all Anthem Blue Cross Plans included a new focus:
- Preventing and managing hypertension.
We’re pleased to offer you this benefit as part of LACERA’s long-standing objective to give all members tools to help them enjoy the best quality of life possible. The Disease Management Programs offered by all LACERA-administered medical plans are voluntary and confidential — we encourage you to take advantage of them.
